Teriparatid is a recombinant human parathyroid hormone (PTH) analog composed of the first 34 amino acids of human parathyroid hormone. It modulates bone metabolism by mimicking the physiological actions of endogenous parathyroid hormone. It intermittently activates the parathyroid hormone receptor (PTH1R), promoting osteoblast proliferation and differentiation while increasing bone matrix synthesis and mineralization. This enhances bone mineral density and improves bone microarchitecture. Teriparatid also regulates calcium and phosphorus metabolism by increasing intestinal calcium absorption and maintaining stable serum calcium levels. Teriparatid is primarily indicated for treating severe osteoporosis, particularly in patients at high fracture risk, significantly reducing the risk of vertebral and non-vertebral fractures. It is also effective for disorders involving abnormal bone metabolism, such as osteomalacia.
